Congratulations, your child has received an offer to attend P.S. 28Q in September 2021. We are collecting student registration information and documents virtually. Don’t worry if you are unable to submit all the required information and documents online, a member of our school staff will contact you to assist if needed.

 

We estimate it will take up to 30-40 minutes to complete the registration forms.

In order to complete the survey you will need to have the following information available:

• Child's name, date of birth, age, address, telephone number, previous school information, OSIS # (found on offer letter),

• Parent(s) name, address, phone number, email,

• Emergency contact person(s) name, address and phone number
   

Once you have gathered the information above and are ready to start the kindergarten registration form complete the forms that were sent to you. Once all forms are completed e-mail them to 24Q028@schools.nyc.gov. If you do not have all the documents available then please complete the registration form without them. We will work with you to submit the forms later on.
